In the first episode of Heavenly Delusion, a group of children dressed in uniforms can be seen playing in a peaceful field surrounded by trees. The scene then transitions to a classroom where a robot teaches the students and conducts a test on a touch screen tablet. One of the students encounters a glitch and a strange question appears, asking if they want to explore "outside of outside," which confuses the student. The focus then shifts to two characters named Kiruko and Maru who explore a ghost town in search of a place they call "heaven." The town is dilapidated, overgrown with weeds, and lacking resources. They attempt to scavenge for resources but are unsuccessful.
While exploring, they are attacked by a group of thugs but manage to defeat them with ease, showcasing Maru's impressive combat skills and Kiruko's ability to create a heat gun. They eventually find an inn where they pay to stay the night and enjoy a warm shower and a tasty pork-based stew. The owner of the inn warns them of a monster that has been terrorizing the area, but Kiruko expresses her desire to defeat it. Later, Kiruko and the owner lose consciousness, and Maru falls asleep early. The owner loads her shotgun as the monster approaches.
Meanwhile, back at the facility, Tokio, who had received the odd "outside of outside" question during the test, is unable to sleep and walks around at night. The director of the facility is also awake and reveals to Tokio the truth about the question, despite being warned not to. She reveals that there is indeed an "outside of outside," but it is a hellish place filled with abominable monsters.

Maru and Kiruko take refuge in the abandoned inn, unaware of the danger that awaits them. While they sleep soundly, the owner arms herself to confront a creature that has appeared outside the building. It remains to be seen whether she will be able to overcome the perilous threat.
