The game features a minimal HUD, and players do not have a health bar, ammunition count, or minimap. Most game modes in the game do not feature instant respawns, and dead players have to spectate and wait for their team to complete an objective before they can respawn. Insurgency: Sandstorm has updated visuals, character customization, cosmetics, weapon skins, a player progression system, larger maps, and drivable technicals.

The game has two opposing teams: Security and Insurgents. Security primarily uses NATO and Western weaponry, while Insurgents use a diverse mixture of weaponry.
