To open the first door leading to the Temple of Bhaal, you'll need to follow these steps:

Locate the entrance to the first door. It can be found near the Undercity Ruins portal. This entrance is part of the sewer system beneath the main city.

Make your way to Bloombridge Park in BG3, where you can find several manholes that provide access to the sewer system.

Once you've reached the first door, you'll notice a circular platform. Above this platform, there are bodies hanging.

Equip a ranged weapon or a spell that allows you to attack from a distance.

Take aim at the bodies hanging above the circular platform and shoot them with your ranged attack. Once the bodies are hit, blood will cover the platform.

As soon as the platform is covered in blood, the doors leading to the Temple of Bhaal will slowly begin to open.

BG3 Temple Of Bhaal Doors

Once you successfully hit one of the bodies and it begins to bleed onto the blood circle, Bhaal's Door will automatically open. Behind the opened door, you will find a lever that can be used to control the door's movements. This lever allows you to open or close the door as needed.
With Bhaal's Door now open, you are able to proceed further into the Undercity Ruins and continue your quest to find the Temple of Bhaal.

The Temple of Bhaal is a long-forgotten and dilapidated structure nestled in the depths of the Undercity, beneath Baldur's Gate. It is an ancient ruin with a rectangular layout, and its entrance can be found on the southern edge of the screen. As you enter, your eyes are drawn towards the grand altar situated at the northern end of the temple.
The interior walls of the temple are adorned with towering statues of robed figures holding scythes, stretching along the western and eastern sides. The eastern wall is further adorned with stone sarcophagi, which pose a dangerous threat as they unleash Battle Horrors when approached. These monstrous creatures can prove to be formidable adversaries.
