Balthazar's Protruding Book Puzzle Guide can be found on the first floor of Moonrise Tower at Balthazar's Observatory. To reach it, you'll need to complete the goblin torture scene in the main hall and then go upstairs.
In the observatory, you'll encounter a bookcase with four rows of books sticking out for you to examine. Alongside these, there are two real bookshelves filled with volumes. Be careful, though, as interacting with the wrong protruding books could trigger a trap. While the lightning bolt and toxic gas effects are temporary, and you can defeat the rising undead, there's a limit to how much your group can endure.
Once you've spoken to Disciple Z'rell upstairs, follow him out of the advisor's Observatory. Then, assign a character with good Sleight of Hand skills to hide and lockpick the door on the top right. This will grant you access to Balthazar's Observatory.
