Lilith corrupts the Druids to locate Astaroth's prison and frees him, in return for a safe passage through Hell. Astaroth possesses Donan's son Yorin, but is eventually defeated, resulting in Yorin's death. Donan retrieves the Soulstone used to trap Astaroth and prepares to modify it in order to imprison Lilith.

Reunited with Lorath in the Dry Steppes, the Wanderer pursues Lorath's former apprentice Elias, who summoned Lilith to Sanctuary. Although Elias cannot be killed, the Wanderer and Lorath acquire an artifact known as the Sightless Eye to uncover Lilith's plan.

Lilith aims to summon the Lesser Evils to empower humanity against the Prime Evils and consume her weakened father Mephisto's essence to conquer Hell. Using a witch named Taissa as a vessel, Elias summons Andariel, the Maiden of Anguish, but she is ultimately defeated by the Wanderer.

In an effort to break Elias' immortality, the Wanderer and the Horadrim travel to the swamps of Hawezar. Elias obtained knowledge from the enchanted "Tree of Whispers" without paying the required price, which was his own head. Learning that Elias has contained his life essence in his severed finger, the Wanderer destroys it and finally defeats Elias.

From the Tree, Lorath discovers a gateway to Hell beneath the city of Caldeum, opened using Rathma's key. In Caldeum, the Wanderer triumphs over another reformed Lesser Evil, Duriel, the Lord of Pain. Inarius ventures into Hell to confront Lilith but is killed by her.

Donan sustains mortal injuries, and Lorath stays behind, leaving the Wanderer and Neyrelle to pursue Lilith to Mephisto's Cathedral of Hatred. Mephisto, appearing as a bloodied wolf throughout the journey, urges them to focus on Lilith, but Neyrelle decides to use the Soulstone to contain Mephisto instead, viewing him as the greater threat. The Wanderer then faces and defeats Lilith.

With both creators of Sanctuary deceased, the party returns home. Neyrelle departs alone with the Soulstone, aware that the Prime Evils are approaching and hoping to discover a way to permanently defeat them.
