Diablo IV unfolds 50 years subsequent to the events of Diablo III: Reaper of Souls, immersing players in the world of Sanctuary. The central antagonist, Lilith, daughter of Mephisto, is summoned by cultists, capitalizing on the weakened forces of demons and angels after previous conflicts.
Thousands of years earlier, Lilith and the angel Inarius collaboratively created Sanctuary, a realm offering respite from the enduring celestial warfare. This union gave birth to the Nephalem, a unique race embodying traits of both Angels and Demons. Sanctuary's inhabitants, fearing the attention drawn by this power, sought to eradicate the Nephalem. In a protective stance, Lilith eliminated those opposing her, leading to her banishment by Inarius.
The player, referred to as "the Wanderer," becomes entwined in Lilith's machinations after ingesting her blood. Encountering Lorath Nahr, a Horadrim, the Wanderer learns of Lilith's impending return. Inarius aims to eliminate Lilith to fulfill a prophecy, intending to ascend to Heaven. Teaming up with Neyrelle, the Wanderer embarks on a journey involving Rathma, the Nephalem founder, and unravels the unfolding events.
The Sightless Eye artifact provides insight into Lilith's grand plan: summoning Lesser Evils to empower humanity against the Prime Evils. Seeking her father Mephisto's power, Lilith's scheme involves absorbing his essence and conquering Hell.
Numerous battles ensue, leading to clashes with Lesser Evils such as Andariel and Duriel. Inarius confronts Lilith, ultimately meeting his demise. Donan and Neyrelle also suffer, while the Wanderer faces Lilith herself, emerging victorious. Lilith's death forewarns of challenges to come.
With Sanctuary's creators gone, Neyrelle takes the initiative, setting out alone with the Soulstone to combat the impending Prime Evil threat. The narrative concludes with the Wanderer and their allies returning home, embarking on a mission to confront the impending Prime Evils and seeking a definitive solution.
