The film begins with a flashback to 1957 when a priest in a church attempts to murder a baby with a unique scar on his chest, considering him the "spawn of evil." However, the priest is interrupted and killed by the arrival of the Milicja (police).
Thirty years later, Father Marek assumes a false identity as a priest and visits a secluded church functioning as a sanatorium for women believed to be possessed by the Devil. Strange occurrences unfold as Marek experiences supernatural phenomena in his room, including rotating crosses, fractured mirrors, and eerie sounds emanating from his wardrobe.
Marek becomes suspicious of the exorcisms performed by Prior Andrzej, particularly after discovering staged effects in one of the rooms. Marek encounters Piotr, who explains the priests' belief that Marek is the "chosen one," destined to fulfill a ritual involving the consumption of flesh and blood. The priests aim to usher in a new era by transforming Marek into a demon. Piotr initially aids Marek's escape but later betrays him to the other priests, leading to Marek's captivity in an underground well.
Awakening restrained at the bottom of the well, Marek endures agonizing pain. The Prior reveals the church's ancient history, explaining their goal of allowing the Devil to rule alongside God. The priests gather and perform a ritual using the blood of a young woman, but it fails to transform Marek as expected. In a panic, Piotr stabs Marek and tosses his body into the well.
The following day, Piotr assumes control of the sanatorium, leading a prayer service for the deceased Prior. However, during the service, Piotr levitates into a crucifixion pose before disintegrating into a swarm of flies.
