In 1924, a man named Jeremy Hartwood, who owned a spooky mansion in Louisiana called Derceto, died by suicide. People already thought the mansion was haunted. You get to play as either Edward Carnby, a detective looking for a piano, or Emily Hartwood, Jeremy's niece who thinks the piano has a secret note about his death. The game starts with your character going to the mansion, and once inside, strange things happen, like doors slamming and monster attacks.

As you explore the mansion, you find out that an occultist pirate named Ezechiel Pregzt built Derceto. He did dark rituals in the caverns under the house to get rich and live longer. The mansion was burned down during the Civil War, but Pregzt's spirit lived on in a tree under the house. Jeremy killed himself to stop Pregzt from using his body, and now Pregzt is after you. If you lose in the game, Pregzt takes over your body, causing supernatural horrors.

In another ending, you find a way to the caverns, throw a lit lantern at Pregzt's tree, and escape as the cavern collapses. This destroys Pregzt, clears the house of monsters, and lets you leave safely. However, the game ends with a twist – when you return to your car, you find out the driver is a zombie who takes you back to civilization.

The story is inspired by H. P. Lovecraft's works, featuring books like Necronomicon, De Vermis Mysteriis, and creatures from Lovecraft's Cthulhu Mythos. The character Edward Carnby's last name is a reference to another character from a Lovecraft-inspired story, and various supernatural enemies in the game come directly from Lovecraft's tales.
