Silent Hill: The Short Message unfolds in the made-up town of Kettenstadt, Germany, focusing on an abandoned apartment complex called the "Villa." Anita, a young woman, gets a message from her late friend Maya, an online graffiti artist known as C.B. (Cherry Blossoms), urging her to meet in the Villa. Anita follows Maya's artworks, facing flashbacks of her own online struggles and bullying. Throughout the game, a cherry blossom-covered monster chases her.

As Anita explores, she receives a call from another friend, Amelie, revealing Maya's death by suicide.
