In the final sequence of the movie "Ma," Sue Ann, also known as Ma, plans another party and lures Ben to her home. Once there, she quickly administers a sedative, diazepam, to him. Sue Ann proceeds to kill Ben in a gruesome manner by transfusing canine blood taken from Maggie's dog into him and then slitting his wrists, causing him to bleed out.

In the basement, Sue Ann spikes the drinks, causing the teenagers to become unconscious. Meanwhile, Maggie, who had been grounded, manages to escape from her home and discovers her unconscious friends and Ben's lifeless body at Ma's house. She too is injected with a sedative and rendered unconscious.

Sue Ann proceeds to torture each of the students, using methods that relate to their past bullying experiences and high school archetypes. The jock, Chaz, is subjected to burns from an iron. The outspoken Haley has her lips sewn shut. The conformist black teenager, Dante, has white paint splashed across his face, with Sue Ann remarking that "there's only room for one of us," making a self-aware comment about the typical portrayal of a single clichéd black character in teen movies.

However, Genie, Sue Ann's daughter, comes to the rescue and incapacitates her mother by hitting her with a frying pan, unintentionally starting a fire in the process. While Sue Ann is temporarily halted, the teenagers regain consciousness and manage to escape from the burning house. When Sue Ann awakens and attempts to drag Genie into the fire, Maggie stabs her and declares, "I'm not like my mother. I'm not weak." At her new high school, Maggie becomes friends with Haley, Darrell, Chaz, and Andy, who quickly develops feelings for her. They persuade Sue Ann, a veterinary technician, to help them buy alcohol. However, Sue Ann anonymously reports them to the authorities, but they are released due to the officer's connection with Andy's father. The following day, Sue Ann invites the teenagers to drink in her basement, and her popularity grows as more students join the parties. Nonetheless, the group becomes annoyed by Sue Ann's persistent attempts to spend time with them. 

One night, she drugs Maggie and steals her earrings, leaving her with cuts and bruises. Frightened, Maggie confides in Andy that she no longer wants to go to Sue Ann's house and doesn't want him to go either. To regain their trust, Sue Ann lies about having pancreatic cancer. Suspecting that Sue Ann has been stealing their jewelry, the girls break into her house to investigate but are caught off guard by Genie, Sue Ann's daughter from a previous marriage. Maggie is surprised to see Genie walking, as she typically uses a wheelchair at school. Maggie and Haley manage to escape as Sue Ann returns home.

Sue Ann's high school acquaintance, Ben, visits her at work and confronts her about his son's visits to her house. It is revealed through a flashback that Sue Ann had been deceived into a sexual encounter by someone pretending to be Ben, which caused her great trauma. Sue Ann seeks revenge and ends up killing Ben's girlfriend, her boss, and injures Ben himself.

She lures Ben to her house, where she ties him up and threatens to harm him. Sue Ann transfuses blood from a dog into Ben, injures him, and leaves him to die. Maggie, a friend, learns the truth about Sue Ann and confronts her briefly. Sue Ann then lures Maggie to her basement, where she realizes that everyone has been drugged. Maggie discovers Ben's body and becomes a victim of Sue Ann's drugging as well.
